The present Report a Concern form is delivered to the site owner. There is not yet an independent safeguarding officer or outside review body. That limitation is disclosed openly because no live center, paid membership, facilitator program, or public gathering is currently operating.
Activities blocked until this exists
The following may not launch under the Living Mirror name until a functioning independent accountability pathway is publicly identified:
- A physical Living Center open to visitors
- Paid membership tied to community participation
- Recurring Witness Circles or facilitated groups
- Facilitator education, authorization, or certification
- Programs involving vulnerable participants or private disclosures
Minimum independent structure
- Independent contact
A named safeguarding professional, qualified advisor, or outside organization that is not controlled solely by the founder.
- Founder-specific reporting
A route for concerns involving Ulysess Ortiz or the site owner that does not send the complaint only to the person named in it.
- Conflict-of-interest rules
Anyone personally, financially, or professionally involved in a concern must step out of decisions about it.
- Written response process
Receipt, initial safety review, documentation, investigation steps, outcome communication, and appropriate outside referral.
- Protection from retaliation
No loss of access, spiritual shaming, harassment, financial punishment, or pressure for reporting in good faith.
- Review and records
Secure records, retention limits, periodic review, and a way to challenge a process failure without promising outcomes that cannot be guaranteed.
What independence does not mean
It does not replace police, emergency services, licensing boards, mandated reporting, legal counsel, medical care, or crisis support. It is a community accountability pathway with clear referral duties and limits.
Public activation standard
When the independent pathway is real, this page must publish the responsible contact or organization, scope, privacy limits, response expectations, conflict rules, and the date the system became active. Until then, the site will not imply that independent review already exists.
